According to the Connecticut Department of Public Health, there are 3 ways to access your unique QR code:
- Visit the CT WiZ Public Portal to get your COVID-19 Vaccination Records with your unique SMART Health Card QR code.
- Call the CT DPH: You can reach CT DPH at (860) 509-7929, Monday through Friday between the hours of 8:30am - 4:30pm. According to the CT DPH, "You will need to submit proof of identification before the printed SMART Health Card is sent to you."
- Visit the DPH's Submissions System, by clicking here, and request for your COVID-19 vaccine card and QR code to be mailed or faxed to you.
