The mandate went into effect on Friday, and failure to comply may result in a $1,000 penalty.
As soon as anyone tries to leave Bradley International Airport, the Department of Public Health is there, waiting with forms by the escalators.
On the travel form, it’s going to ask travelers a series of questions such as: Have you traveled to an affected state, are you an essential worker on work-related travel, where and how long do you plan to stay while in Connecticut.
The form also asks travelers to submit their name, date of birth, state of origin, as well as contact information.
There are a few ways people can complete the contact tracing form.
